PSA crept up to 3.8 on TRT, do I actually need to see a urologist now

Just got bloods back, PSA is 3.8. I am 47, been on TRT for 3 years at 150mg test e per week. My GP flagged it and mentioned referring me to urology. Googled it and 3.8 is technically in the elevated range but I have read on here that TRT always bumps PSA a bit. Should I be actually worried or is this just one of those numbers that looks worse than it is?

Rate of change is what matters, not the absolute number. What was your PSA a year ago? If it went from 2.1 to 3.8 in 12 months that is worth taking seriously. If it was 3.4 last year and 3.8 now, that is within normal variation on TRT. Get at least one previous PSA reading to compare before panicking. And yes see the urologist, not because you are definitely in trouble but because it gives you a baseline and rules things out.

PSA velocity is the clinical metric that matters. Anything above 0.75 ng/ml per year is the threshold urologists use to recommend biopsy. At 47 on TRT a PSA of 3.8 is elevated but not alarming in isolation. High DHT compounds like Masteron and Proviron are more likely to push PSA than testosterone alone - are you running anything else? Drop to cruise-only for 6 weeks and retest before seeing the urologist, gives you cleaner data.

A mate of mine got flagged at 4.1 PSA on TRT, saw the urologist, biopsy came back completely clear. His urologist actually said TRT patients commonly sit in the 3-5 range and the concern is really rapid rises or sustained high numbers. Annual tracking from 40 onwards is just good practice if you are on long term TRT. Do not skip the referral though, the peace of mind is worth it.
